Copy Form & Get Published URL
Click below to copy our form template to your Google Drive.
Copy Form TemplateAfter copying the form:
1. Your copied form will open
2. Click the "Publish" button (top section of the form)
3. A dialog will appear with your form URL
4. Copy this URL - you'll paste it in Step 2 below
⚠️ Important: You MUST publish and copy this URL before proceeding to Step 2!
Paste Form URL & Create Google Sheet
First, paste the published form URL from Step 1. Then create your Google Sheet.
Paste the URL you copied from the Publish dialog in Step 1
Now create your Google Sheet:
1. Go back to your form (open it in a new tab if needed)
2. Click the "Responses" tab
3. Click the green button: "Create New Google Sheet"
4. A new Google Sheet will open
5. Copy the URL from the address bar of the sheet
6. Paste it below
Paste the URL from your Google Sheet's address bar
Manage Tags (Optional)
Enable or disable tags for organizing your saved pages. Select up to 4 tags when saving.
